A new community Health and Wellbeing Centre

End 2021, Bromley by Bow Centre (BBBC) was commissioned to support the articulation of a ‘Model of Care’ (MoC) to underpin the further development of a new Health and Community Centre for one of London’s most deprived wards. This centre is being considered since as far back as 2013.

Model of Care

Traditionally, Models of Care have sought to describe systems for the effective treatment and management of patients with diagnosed conditions in various stages of advancement.

The BBBC ‘Model of Care’ approach, however:

  • Expands the traditional Model of Care to incorporate a more preventative and holistic approach to health and wellbeing in a ‘whole community’ sense,
  • Embraces the Marmot Reviews 2010/2020 (‘Health inequalities result from social inequalities’), and
  • Adopts an asset-based approach grounded in:
    • the principles of community development (strengthening relationships, networks, and capacity building), and
    • improving the social determinants of health.

Model of Care Readiness

FVD Consulting assisted in the research to assess the centre’s Model of Care ‘readiness’ and concluded that:

  • There is sufficient information available to answer the Who? question (Who are the community? What are their needs and opportunities?); but
  • There is insufficient definition of the Vision and Strategic Objectives for the proposed centre (The Why? question – What is the shared vision for the community?)

The research briefing document, written in collaboration with KYMA Consulting and William Nicholson, independent health and wellbeing consultants, proposes:

  • A draft Vision (‘Opportunity Statement’)
  • Design for a workshop with senior representatives from the local authority and the NHS to agree a draft set of strategic objectives
  • Designs for four sector-specific workshops aiming to develop a common level of understanding and identification with the opportunity that the project presents
  • Preliminary outline for two further collaborative workshops
  • Next steps
